چکیده
BACKGROUND Progressive narrowing of the venous part of dialysis fistulae is caused by hemodynamic and inflammatory factors. OBJECTIVES The pathogenic and clinical determinants of deterioration of the functioning of arteriovenous fistulae in chronically hemodialyzed patients were evaluated. MATERIAL AND METHODS The hemodynamic parameters and the activity of inflammatory growth factors in the vessel wall of newly implanted fistulae were assessed and correlated with the clinical course of 34 hemodialyzed patients. Measurements taken at the time of implanting the fistulae included blood flow in the venous part of the anastomosis and its widest diameter by ultrasound Power Doppler, a histopathologic examination of fistula wall samples and measurements of mRNA expression for growth factors PDGFβ1 and TGFβ in the fistula wall. The results were correlated with clinical data from 36 months' observation: duration of fistula maturation, adequacy of dialysis treatment (eKt/V), the patient's survival, morbidity linked with vascular access problems and general cardiovascular morbidity. RESULTS The mean duration of fistula maturation was 44.9 days (N = 43, SD = 38.6), whereas the average duration of fistula usage as dialysis access was 795.9 ± 480.6 days. Fistula blood flow at the time of implantation, averaged 1782.2 ± 1735.3 ml/min. The mean number of hospitalization days due to vascular access morbidity was 9.9 ± 15.6 days and it correlated positively with the fistula blood flow (R = 0.596, P = 0.004). There was a negative correlation between the expression of PDGFβ1 mRNA and fistula blood flow (R = -0.673, P = 0.011), as well as between TGFβ expression and patient survival (R = -0.722, P = 0.002). CONCLUSIONS Inflammatory activity of the vessel wall growth factors PDGFβ1 and TGFβ implies impairment of fistula function and the patient's cardiovascular morbidity.

Data Mining Performance in Identifying the Risk Factors of Early Arteriovenous Fistula Failure in Hemodialysis Patients
Background and Objectives: Arteriovenous fistula is a popular vascular access method for surgical treatment of hemodialysis patients. The method, however, is associated with a high rate of early failure varying in the range of 20-60%. Predicting early Arteriovenous fistula failure and its risk factors can help reduce its incidence, its hospitalization rate, and associated costs. In this study, ...
متن کاملComparing the Effect of Cryotherapy and Acupressure on Pain Intensity of Arteriovenous Fistula Cannulation in Patients Hemodialysis
Introduction: Patients with kidney failure undergoing hemodialysis are frequently exposed to the hemodialysis needles are inserted into the arteriovenous fistulas; one of the important interventions in nursing care is use of nonpharmacological interventions for relief pain. Therefore, this study was conducted to compare the effect of cryotherapy and acupressure on the pain intensity of needle ...

Background: Hemodialysis is the sole way for management of patients with End Stage Renal Disorders (ESRD). Nowadays, inserting arteriovenous fistula is one of the most common methods for effective hemodialysis. Several factors have been reported for failure of arteriovenous fistulas such as diabetes, biochemical factors, low hemoglobin and increased level of blood calcium and phosphorus. Decisi...
متن کاملEffect of Mirror Therapy on Arteriovenous Fistula Cannulation-Related Pain Severity in Hemodialysis Patients
Background: Hemodialysis patients experience pains induced by cannulation of an arteriovenous (AV) fistula. The effect of mirror therapy on patients’ pain severity has not been investigated in individuals living with hemodialysis. Aim: The purpose of the present study was to investigate the effect of mirror therapy on AV fistula cannulation-related pain s...

Background: Arteriovenous Fistula (AVF)is the ideal method of vascular access for patients on maintenance hemodialysis (HD).Therefore it is an important part of treatment in HD.There are several observations that indicate the role of inflammation in failure of AVF.The aim of this study was to evaluate the hematologic and inflammatory biomarkers in early AVF failure. Methods : This case-cont...
متن کاملپیشبینی شکست زودرس فیستولهای شریانی- وریدی بر مبنای معاینه در خاتمه جراحی
Background: For patients requiring chronic hemodialysis, the preferred site for vascular access is an autogenous arteriovenous fistula. Although a properly formed fistula is advantageous because it is less susceptible than other types of vascular accesses to infection and clot formation and can last longer than any other types of vascular access, AV fistula has a high rate of early failure that...
